Booking and consent form

You will have signed this form automatically, if you've booked in via our bookings calendar. If you have made a booking on behalf of other people, please make sure they fill out the form before the session, so that no time needs to be wasted on paperwork on the day. This can be done in two ways:

Each individual creates a user account in our online booking system. This is the preferred method, as they are then automatically elibigle for a discounted returning paddlers rate on future group SUP trips.  Please ask them to go to our online calendar at Click ‘Log in’ at the top right corner of the calendar screen and then choose “Create new user account”. This takes you through the consent form. After you receive the confirmation email, click on the link to verify the user account. Log in, locate the Sunday of the week of your session in the calendar and find the 9pm session called “ACTIVE360 PADDLER”. Click on “New booking”. Your details should be auto-populated. Simply click “Create booking” and you’re done.

Alternatively, ask them to download the form here, fill it out and either email it back to us or bring it with them on the day.

equipment hire agreement

You will need to provide this when collecting equipment to hire. It can be emailed back to us, or brought with you on the day.
